The Health Determinants Planning & Evaluation Division was created in 1999 to provide policy, research, and evaluation support to other Public Health divisions and the community, with a focus on the social, economic and environmental conditions that determine health.

Our strategy to do this is using an integrative population health approach that focuses on the determinants of health and emphasizes capacity building and participative processes.

We are organized under three program teams:

• Epidemiology
• Planning & Evaluation
• Healthy Communities & Policy

• Epidemiology and Knowledge Management – increasing the capacity of staff and the community to effectively manage and use health data and information.
• Planning & Evaluation – increasing evidence-based and integrated planning and evaluation that is relevant to public health.
• Access & Equity – reducing educational, social, and environmental barriers to accessing mandatory public health programs
• Health and the Built Environment - Increasing Liveability – improving the design of the built environment in order to improve related health outcome.
• Citizen and Community Engagement – increasing institutional and community capacity to engage citizens toward the creation of public health programs and policies.
• Community Food Systems – increasing access to safe, nutritious food that is grown in a sustainable way.
